Il-62 accident in Yakutsk

Causes

The accident involving Il-62M aircraft No. 86456 was the result of the following deviations in the crew's actions:

errors by the flight engineer in controlling the engines and spoilers during the landing, which led to the unintentional and unexpected selection of forward takeoff thrust on engines 1 and 4 instead of reverse thrust, which caused the aircraft to accelerate after touchdown, blocked the extension of the spoilers and reduced the effectiveness of the main braking system;

the crew's lack of organisation in containing the consequences of the flight engineer's erroneous actions.

These deviations in the crew's actions were caused by a combination of the following factors:

The heavy verbal workload of the crew during landing, as prescribed by the current "Instruction on crew coordination and work procedures for crew members of the Il-62M, Il-62 aircraft" of 12.02.82.

Insufficient professional skills of the flight engineer in operating the reverse thrust levers and throttle levers when selecting reverse, and lack of teamwork between the aircraft commander and the flight engineer.

Failure by the command and flight staff of the DPO to implement the recommendations of the USSR Ministry of Civil Aviation of 24.08.87 on forming crews taking into account psychophysiological and socio-psychological compatibility, as well as the instruction of the USSR Ministry of Civil Aviation of 16.05.86 No. 310/U regarding the practising of crew actions in emergency flight situations under aerodrome conditions.
