Cause areas

  • Pilot in command › Aborted takeoff, delayed
  • Pilot in command › Airspeed, not attained

Approximate · Coordinates as recorded by the NTSB; no uncertainty radius is established.

NTSB narrative

THE PILOT SAID THAT WHILE ATTEMPTING TO TAKEOFF, ON THE 1485 FOOT LONG GRASS RUNWAY, HE WAS UNABLE TO ATTAIN FLYING SPEED. HE ABORTED THE TAKEOFF, BUT WAS UNABLE TO STOP BEFORE COLLIDING WITH A DIRT BANK.

Probable cause

AIRSPEED WAS NOT ATTAINED AND ABORTED TAKEOFF WAS DELAYED BY THE PILOT. FACTORS RELATED TO THE ACCIDENT WERE: OVER CONFIDENCE IN AIRCRAFT'S ABILITY TO TAKEOFF ON THE SHORT, GRASS RUNWAY AND A DIRT EMBANKMENT ON THE DEPARTURE END OF THE STRIP.

  • Unreviewed is an editorial label, not an investigation status. API-sourced is not report-checked or human-reviewed. Explicit event totals are used without summing aircraft injury tables; onboard allocation is withheld. Unknown values remain unknown. The operation category is mapped from the NTSB-reported FAR part and has not been reviewed.
  • Filled from the NTSB case API where the bulk record had no value: local date, investigation status. Values present in the bulk record are kept.
  • The date is the local date; the NTSB stores the UTC date 1991-06-11.
