Accident
Carrying out a private flight, the airplane took off from Comandante Rolim Adolfo Amaro Aerodrome, bound for Barra Grande Aerodrome, in Maraú, Bahia, Brazil, at around 14:58 (UTC), with eight passengers on board, in addition to the pilot and co-pilot. Arriving at the destination aerodrome, at 17:17 (UTC), the aircraft landed short of runway 11, causing the rupture of the main and auxiliary landing gear. One crew member and four passengers suffered fatal injuries and one crew member and four passengers suffered serious injuries.
Investigation and cause
Investigation
The final report released by the Aeronautical Accidents Investigation and Prevention Center (CENIPA) on 19 August 2021 points to the following contributing factors:
Control inputs — which led the aircraft to fly a lower approach slope than ideal;
Cockpit coordination — with inadequate use of Crew Resource Management; and
Piloting judgement — inadequate assessment by the captain of the aircraft's position.
As possibly contributing factors (undetermined), the following were identified: Attention, Attitude, Communication, Visual illusions, Perception, Flight planning and Physical-sensory limitations.
