NTSB narrative

SCHEDULED AIR CARRIER EXPERIENCED APU FIRE WARNING WHEN APU START WAS ATTEMPTED DURING APPROACH DESCENT. INVESTIGATION REVEALED THAT EGT TEMPERATURE PROBE WIRING HARNESS WAS PINCHED BETWEEN FIRE SEAL AND LOWER APU SHROUD. FIRE SEAL COMPROMISE ALLOWED POOLED FUEL TO ENTER LOWER APU AREA AND CAUSE LOCALIZED FIRE ON DELAYED IGNITION.

Probable cause

FAILURE TO ENSURE PROPER ALIGNMENT AND CLOSURE OF THE AUXILLARY POWER UNIT LOWER SHROUD WITH THE FIRE SEAL UPON COMPLETION OF A MAINTENANCE ACTION.

  • Unreviewed is an editorial label, not an investigation status. API-sourced is not report-checked or human-reviewed. Explicit event totals are used without summing aircraft injury tables; onboard allocation is withheld. Unknown values remain unknown. The operation category is mapped from the NTSB-reported FAR part and has not been reviewed.
  • Filled from the NTSB case API where the bulk record had no value: investigation status, damage. Values present in the bulk record are kept.
  • The date is the local date, which is the same as the UTC date the NTSB stores.
  • API snapshot SHA-256: da8becb67924b692830b2cb7404cdc097387360870ce2e09753231a09d9299a7; retrieved 2026-09-18T19:20:51.253Z.
  • The NTSB's operator name is blank for N409PE; the operator shown is the name the NTSB's other records give its FAA operator code, CALA.
