NTSB narrative

DURING CRUISE FLIGHT AT 12,000 FT, THE FLIGHT CREW OF THE DHC-8 OBSERVED A LOW OIL PRESSURE WARNING LIGHT. ABOUT FIVE SECONDS LATER, THE RIGHT ENGINE FIRE WARNING LIGHT ILLUMINATED. FIRE WAS CONFIRMED AND THE EXTINGUISHER DISCHARGED. THE FIRE WAS EXTINGUISHED BUT FLARED UP AGAIN, WHEREUPON THE SECOND BOTTLE WAS FIRED. THE FIRE WAS EXTINGUISHED AND A LANDING COMPLETED WITHOUT FURTHER INCIDENT. INSPECTION REVEALED BROKEN OIL LINES TO THE NUMBER 6 & 7 BEARINGS. FURTHER INVESTIGATION REVEALED FAILED LOW PRESSURE TURBINE BLADES DUE TO INTERNAL CORROSION. NEW SOLID BLADES ARE UNDER ENGINEERING EVALUATION ACCORDING TO THE MANUFACTURER.

  • Unreviewed is an editorial label, not an investigation status. API-sourced is not report-checked or human-reviewed. Explicit event totals are used without summing aircraft injury tables; onboard allocation is withheld. Unknown values remain unknown. The operation category is mapped from the NTSB-reported FAR part and has not been reviewed.
  • Filled from the NTSB case API where the bulk record had no value: investigation status. Values present in the bulk record are kept.
  • The date is the local date, which is the same as the UTC date the NTSB stores.
