NTSB narrative

THE PIC/CFI WAS CONDUCTING A PUBLIC USE INSTRUCTIONAL FLIGHT WITH TWO MILITARY RATED STUDENT PILOTS. THE AIRPLANE WAS OBSERVED AT ABOUT 60 TO 80 FT AGL SOUTH OF THE APPROACH END OF AN ACTIVE RUNWAY. IT YAWED TO THE RIGHT, AND BACK TO THE LEFT, COLLIDING WITH THE TERRAIN IN A 45 DEG LEFT-WING-LOW ATTITUDE, AND DISAPPEARED FROM VIEW. THE FLIGHT HAD BEEN IN THE PATTERN MAKING TOUCH-AND-GO'S, AND HAD EARLIER REPORTED A SINGLE-ENGINE APPROACH. THIS LANDING WAS TO BE A FULL-STOP.

Probable cause

THE PILOT-IN-COMMAND/CFI IMPROPER SUPERVISION OF THE DUAL STUDENT. THIS RESULTED IN AN IN FLIGHT LOSS OF CONTROL, AND COLLISION WITH TERRAIN FOR FAILURE TO MAINTAIN MINIMUM CONTROL AIRSPEED (VMC) WHILE CONDUCTING A SIMULATED EMERGENCY PROCEDURE.

  • Unreviewed is an editorial label, not an investigation status. API-sourced is not report-checked or human-reviewed. Explicit event totals are used without summing aircraft injury tables; onboard allocation is withheld. Unknown values remain unknown. The operation category is mapped from the NTSB-reported FAR part and has not been reviewed.
  • Filled from the NTSB case API where the bulk record had no value: investigation status. Values present in the bulk record are kept.
  • The date is the local date, which is the same as the UTC date the NTSB stores.
  • API snapshot SHA-256: 1df2c0fb59064201aa90d0f6c770b4820ff5e78f5847b7a8a420bd967b5d4f05; retrieved 2026-09-21T02:05:15.068Z.
  • Corrected after a review of the NTSB narrative, which states these values explicitly: operation. The narrative states "THE PIC/CFI WAS CONDUCTING A PUBLIC USE INSTRUCTIONAL FLIGHT WITH TWO MILITARY RATED STUDENT PILOTS"; the flight was a public aircraft operation, not Part 91 general aviation.
