NTSB narrative

THE FLT DEPARTED CHEHALIS, WA, ON A VFR FLT TO BOUNDRY BAY, CD, AND THE PILOT LANDED AT PUYALLUP, WA, WHEN HE ENCOUNTERED ADVERSE WEATHER EN ROUTE. AFTER A DELAY, THE PILOT TOOK OFF TO CONTINUE THE FLT. THE ACFT RETURNED TO THE ARPT AFTER A FEW MINUTES IN FLT AND ENTERED THE TRAFFIC PATTERN FOR LANDING. WHILE ON FINAL APPROACH AT A LOW ALTITUDE AGL, THE ACFT STALLED, DESCENDED NOSE DOWN AND IMPACTED THE GROUND AT A PRIVATE RESIDENCE. THE FLAPS WERE IN THE UP POSITION IN THE WRECKAGE.

Probable cause

THE FAILURE OF THE PILOT TO CONFIGURE THE ACFT FOR LANDING (LOWERING OF FLAPS) AND MAINTAIN AIRSPEED ABOVE STALL DURING FINAL APPROACH FOR LANDING.

  • Unreviewed is an editorial label, not an investigation status. API-sourced is not report-checked or human-reviewed. Explicit event totals are used without summing aircraft injury tables; onboard allocation is withheld. Unknown values remain unknown. The operation category is mapped from the NTSB-reported FAR part and has not been reviewed.
  • Filled from the NTSB case API where the bulk record had no value: local date, investigation status. Values present in the bulk record are kept.
  • The date is the local date; the NTSB stores the UTC date 1992-09-21.
