NTSB narrative
DURING AN APPROACH TO A HELIDECK THE PILOT EXPERIENCED A PARTIAL POWER LOSS AND SUBSEQUENTLY MADE A SUCCESSFUL AUTOROTATION. DUE TO HIGH SEA STATE THE PILOT ELECTED TO MAINTAIN IDLE POWER IN ORDER TO AVOID TAIL BOOM CONTACT WITH THE MAIN ROTOR BLADE WHILE AWAITING RESCUE. WIND WAS FROM 300 DEGREES AT 18 KNOTS, GUSTING TO 30. WAVE ACTION DAMAGED THE TAIL ROTOR BLADES WHICH THEN CONTACTED THE TAIL BOOM. AT THAT POINT ALL OCCUPANTS BOARDED A LIFE RAFT. AN ENGINE TEST RUN WAS SUCCESSFUL. THE CAUSE OF THE POWER LOSS WAS UNDETERMINED.
Probable cause
HEAVY SEA STATE DUE TO THE HIGH WINDS. A FACTOR WAS THE POWER LOSS.

- Unreviewed is an editorial label, not an investigation status. API-sourced is not report-checked or human-reviewed. Explicit event totals are used without summing aircraft injury tables; onboard allocation is withheld. Unknown values remain unknown. The operation category is mapped from the NTSB-reported FAR part and has not been reviewed.
- Filled from the NTSB case API where the bulk record had no value: investigation status. Values present in the bulk record are kept.
- The date is the local date, which is the same as the UTC date the NTSB stores.
